Medication and Allergy Information

​Saint Anselm’s Academy is dedicated to providing a fun, safe and engaging environment to all children.  Our programs are licensed by New York State and adhere to all requirements that accompany licensure.

If your child has any allergy, such as food (peanuts, tree nuts, eggs, milk, etc.), medication (amoxicillin, penicillin, etc.), environmental (dander, bee stings, etc.), medical forms must be completed and all
medication must be provided to Saint Anselm’s Academy before your child’s first day of school. These forms are required for children enrolled in any program at Saint Anselm’s Academy For all children under the age of 5 years old, these forms will need to be updated every 6 months.

According to the regulations of the NYS Office of Children and Family Services (OCFS), if the use of medication is indicated by a child’s pediatrician/allergist, Saint Anselm’s Academy must have written permission and written directions from both the parent/guardian and the medical provider to administer medications.
  1. An Individual Health Care Plan (OCFS-LDSS-7006) must be completed in advance by the parent/guardian and reviewed and signed by a Saint Anselm’s Academy administrator.
  2. An Individual Allergy and Anaphylaxis Emergency Plan (OCFS-6029)
  3. A Medication Consent Form (OCFS-LDSS-7002) must be completed and signed by the child’s pediatrician/allergist, parent and a Saint Anselm’s Academy administrator for each medication your child requires at school. For example, if your child needs to have Benadryl and EpiPen, two Medication Consent Forms must be completed.
  4. All prescription medication brought to Saint Anselm’s Academy must be in its original box with the original pharmacy label. Over the counter medications must be unopened and labeled with the child’s name on it. Medication samples will not be accepted. Please make sure to check the expiration dates of medications to ensure they are usable. 

If your child has an allergy that does not require medication (for example, amoxicillin), the following forms must be completed and updated every 6 months for children under the age of 5 years old:
  1. An Individual Health Care Plan (OCFS-LDSS-7006) must be completed in advance by the parent/guardian and reviewed and signed by a Saint Anselm’s Academy administrator.
  2. Individual Allergy and Anaphylaxis Emergency Plan (OCFS-6029) must be completed and signed by the child’s pediatrician/allergist, parent and a Saint Anselm’s Academy administrator.
